CareBridge helps people stay at home with 24/7 access to a care team (nurses, therapists, social workers). They work through many Medicaid and Medicare health plans — not a walk-in clinic you call on your own. Ask the health plan if CareBridge is included.
They may send a tablet or app so the member or caregiver can reach help day or night, including a “red button” for urgent clinical support.
